The Healthy Hearts Northwest is a three-year project funded by the Agency for Healthcare Research and Quality (AHRQ) that helps primary care practices in Washington improve their patients’ cardiovascular health – while also building their capacity for quality improvement (QI). The program will resolve around Aspirin use, Blood pressure control, Cholesterol management, and Smoking cessation. (ABCS)
